To Qualify: Must be over 60 years

1. Fill out application
2. Provide proof of household income. This includes a copy of your new Benefit Letter from Social Security. If you do not have a copy call: 1-877-846-8333
(No Bank statement will be accepted)
3. A photo ID to prove your age
4. A copy of a bill or an envelope with your address on it.
Monthly Household Income Cannot Exceed:
1 person household - $1,580/month
2 person household - $2,136/month
