Shoutout to one of the kindest and most compassionate professionals I know—Kim at Comforting Transitions, LLC, a dedicated care manager who truly sees and supports older adults with love, dignity, and grace. If you or someone you know needs guidance navigating aging care, she’s the gentle hand and steady heart you want by your side.
So grateful for the work she does and the trust we share in helping families through life’s important transitions.
If you or someone you love is navigating a new dementia diagnosis, it can feel overwhelming. You may be asking: Where do we start? What does the future look like? How do we make the right decisions for care and quality of life?
As a member of the Aging Life Care Association, I help families answer these questions with confidence and compassion. I have 17 years experience as a home health RN, caring for elderly patients and has Master’s in Nursing and Science as well as being board-certified as an Adult-Geriatric Nurse Practitioner through the American Association of Nurse Practitioners (AANP). In addition, we follow a strict Code of Ethics and Standards of Practice to guide our work.
This helpful guide from ALCA explains how professionals like me can support families through the many challenges that come with a dementia diagnosis. From understanding the condition to coordinating care and advocating for your loved one, we are here to walk with you every step of the way.
