Passenger Profile/Health Declaration (PPHD)
Please be advised that if you are departing from the Philippines, you must register and accomplish the Passenger Profile and Health Declaration (PPHD) Form and save the confirmation e-mail and QR code, as early as 5 days prior to your departure. This is a 2-step pre-registration and registration process. Click this link to register: https://bit.ly/PALIntPPHD
- For dependents with valid resident permit, select Filipino traveler under passenger type.
- For passengers holding work visa in Qatar, select Land based OFW under passenger type.
- For passengers holding seaman crew visa and seaman’s passbook, select Sea-based OFW under
passenger type.
- For passengers holding Student visa, select Student /Participant under passenger type.
- For foreign passengers holding Permanent Resident visa in the Philippines, select Citizen/Permanent
Resident of foreign jurisdiction under passenger type.
- For passengers holding foreign passports, select Foreign National under passenger type
The online portal will be open for registration on November 10, 2020 for flights starting November 12, 2020.
Exempted from accomplishing the PPHD form online are travelers bound for the United States, Canada and Australia, all of whom must accomplish the paper form upon airport check-in.
Passengers bound for Singapore are required to accomplish the PPHD paper form upon airport check-in in addition to the online registration.
Local Manning Agencies (LMAs) or Manpower Agency may register on behalf of their passengers.

RT-PCR TEST is not required for Qatar bound passengers.
