1.) Valid Photo ID - driver's license, company id, passport, sss, gsis, prc (you need to leave pertinent ID with us.)

2.) Proof of Financial Capacity/Proof of Billing Address (latest copy) - credit card statement, bank statement, public utility bill (meralco, maynilad, pldt, bayantel & others), or computerized payslip from reputable company.

3.) You will also be photographed (close-up and whole body) for our record and security purposes (a must).

4.) Php10,000 insurance refundable deposit
