WHAT DOES A FULLY COMPLETED VACCINE CERTIFICATE LOOK LIKE?
To complete an AHC I need to have proof of rabies vaccination and proof that the microchip was read on or before that date of rabies vaccination. This can be in the form of an old pet passport or relevant medical history emailed to me by your vet or as a vaccine certificate/card.
Below is an example of what a vaccine card may look like, they come in many different styles and it doesn’t matter what format yours is in as long as it contains the following information -
1. Your details
2. Your pets details
3. Details of rabies vaccination including date given, manufacturer and batch number of vaccine and date next vaccine due
4. Microchip number and very importantly a note to say the date the microchip was implanted or read - this is usually put down as reading on the date of rabies vaccination
5. Vet details - signature, name and practice details
The things that are most often missing on these cards are the full vet details and the date of microchip reading.
