The crew cleaned the vessel's cargo holds in preparation for the next cargo while the ship was underway with calm sea conditions. To access the upper areas of the hold, they used high pressure washing equipment on portable staging secured to the ship's structure. Unfortunately, a crew member fell from the staging while it was being re-positioned, resulting in serious injuries. The vessel had to deviate to evacuate the injured crew member via helicopter.

Cargo hold fall incident :
The crew received instructions to clean the cargo holds of the vessel in preparation for the next cargo. At the time, the vessel was underway, experiencing slight sea conditions with no noticeable ship movement. To facilitate better access to the upper areas of the hold, the crew set up high-pressure washing equipment on portable staging erected on the tank top. This staging, about 5m high, was securely lashed to the ship's structure to ensure stability. Additionally, a safety line was installed across the hold, allowing the seaman atop the staging to attach a safety harness. When it became necessary to relocate the staging, the crew loosened the lashings and pushed the apparatus across the tank top. Unfortunately, during this maneuver, the seaman lost balance and fell onto the tank top, sustaining severe injuries. As a result, the vessel altered its course to facilitate the helicopter evacuation of the injured crew member.
